Responsibilities

  • You will be treated as an individual in the care and attention that you receive and should be shown courtesy and respect.
  • Following discussion you will receive the most appropriate care, and this will be given by a suitably qualified member of staff, no care of treatment will be given without your consent.
  • Be referred to consultant when their GP or Nurse thinks it necessary and to be referred for a second opinion if they and their clinician agree to this.
  • Have access to their own Health records, subject to any limitations of the law, and to know those working under the NHS have a legal obligation to keep their confidentiality.
  • The Practice operates an open list policy and any patient wishing to join the practice will not be discriminated against.
  • All patients will be treated with equality, dignity and respect.
  • Choose whether or not to take part in medical research or student training.
  • Any patient between the ages of 17 and 74 years are entitled to a health check if they so desire. Annual health checks for the over 75’s if necessary.
  • Receive full and prompt reply to any complaint made regarding the NHS.
  • Have a named GP.
  • Children under 5 will be seen on the same day.
  • Over 75s, patients with learning disabilities and carers are given a dedicated number to ring on registration.
  • All patients are provided with online passwords to access repeat prescribing, appointments and access to medical records.
